What is a Data Use Agreement (DUA)?
A Data Use Agreement (DUA) is a required legal agreement. It allows data to be shared between LSUHS and an external institution or party while ensuring the data is protected under HIPAA and LSUHS policies.
A DUA explains:
- What data is being shared
- How the data can be used
- Who can use or receive the data
- How privacy and legal protections will be ensured
What is the Role of the Office for Innovation & Commercialization (OIC)?
OIC helps faculty and researchers with all incoming and outgoing DUAs.
- Coordinates and tracks DUA requests
- Assesses intellectual property (IP) considerations
- Ensures necessary compliance reviews (e.g. HIPAA, Privacy)
- Routes agreements for legal review and signature
- Maintains records of DUAs for institutional tracking

► You must complete this form for any data shared with or received from an external party, even if the other institution says a DUA is not required. This ensures LSUHS reviews data sharing for compliance, legal, privacy, and IP considerations.
► Data sharing agreements can only be signed by official institutional signatories or administrators authorized by LSUHS. Researchers and staff are not permitted to sign agreements on behalf of the institution.
